New Student Registration

All students who have never attend
ed a District 160 school must register at the school that they will be attending. Country Club Hills School District 160 has three grade level centers:

  1. Official Birth Certificate - NOT A HOSPITAL CERTIFICATE (Available from office in county where child was born.)
  2. Proof of Identification and Residency - Please click here for the list of acceptable documentation.
  3. Parent’s home, work, alternate phone numbers and an email address
  4. Two (2) emergency contact names and phone numbers (other than parents/guardians)
    • Current signed lease agreement; and
    • Proof of current month’s rent payment; and
    • Utility bill in your name (water, natural gas or electric) or a driver’s license with current address
  5. Proof of guardianship (if child is not living with birth parents)
  6. Illinois Certificate of Physical Health Exam Form (must be submitted and approved before child can start school)*
  7. Transfer students only- State of Illinois Transfer Form

 If you have questions about required documentation or other registration and enrollment procedures, please do not hesitate to contact the school office. If you do not have access to a computer to complete the online portion of the registration process, computer access will be made available to parents and guardians at the school buildings on the following dates:

STEP 2 of the Registration Process: Provide School office with identification and residency verification documents, a copy of child’s Official Birth Certificate and the completed Illinois Certificate of Physical Health Exam Form. Proof of residency must be completed prior to enrollment and attendance at school. A copy of the Illinois Certificate of Physical Health Exam Form is available on this website.
*The deadline to provide a completed Certificate of Physical Health Exam Form to school office for processing is Monday, August 1, 2016.

School offices will be open on the following dates and times to complete step 2 of the registration process:

 Tuesday, July 5- Monday, August 1, 2016.

STEP 3 of the Registration Process: Parents/guardians will be notified once required information is verified and the child is officially enrolled.
Children may begin school only after this process is complete.

STEP 3 of the Registration Process*: Provide the school main office with a completed and signed copy of the Illinois Certificate of Physical Health Examination Form (*required for children entering Preschool for the first time, Kindergarten and 6th grades). Deadline to provide Certificate of Physical Health Examination Form: Monday, August 1.

All school fees must be paid through the online registration portal. If you do not have access to a computer to complete the online portion of the registration process, computer access will be made available to parents and guardians at the school buildings on the dates and times listed above.

 *First Day Exclusion Policy

It is a state of Illinois requirement that all children entering Preschool, Kindergarten and 6th grade submit a completed Certificate of Physical Health Examination Form (including documentation of all required immunizations).  If a child’s required physical and immunizations are not submitted prior to the first day of school, delay in enrollment and school attendance may occur. All physical and immunization forms must be reviewed and certified by the school nurse prior to the student being enrolled and attending school. Transfer students may have different requirements, please contact the school office for additional information. Prior to submitting your child’s completed medical form(s), parents are advised to make a photocopy for their records, as copies will not be made at the schools or District Office.
